We use our effective partnerships with other end of life care services in the community to ensure your coordinated care, giving you, and your family space and time to focus on normal, everyday life. Our highly skilled care workers may even undertake delegated tasks on behalf of district nurses and co-deliver neuro rehabilitation programmes to reduce your need for hospital appointments, minimising any impact to you.

 

Our end of life care workers in Newcastle are trained to provide you and your family with emotional support, making direct referrals to counselling services and other local community services that can offer further help.
