We also know that it can be hard to be fully open with someone unfamiliar so we use an ‘about me’ form for you to complete which we review and continually add to as we get to know you better.

 

Meeting your needs, choices and preferences

We’ll listen to you at all times, respecting your freedom of choice to adapt your care plan day-to-day. Our aim is to maintain your dignity and meet your spiritual and religious wishes - allowing you and your family to enjoy time together without having to worry about pain management, personal care and other areas in which we support.

 

We know how important it is for you to choose the kind of care you receive, to provide continuity for you and ensure we know about your needs as they develop. We help make you aware of your options and ensure all your choices and preferences are voiced and recorded.

 

Your preferred place of death is recorded during planning. For many of our customers this is usually their own home, which we facilitate by coordinating care delivery with our partners and arrange home visits for all medical appointments. In certain circumstances there may need to be a deviation from your plan - this will always be explained, discussed and decided by you and your family.

Planning support for every aspect of your life

We ensure the needs of your family are not overlooked during your care delivery and afterwards. Our end of life care workers are trained to provide emotional support, making direct referrals to counselling services if and when needed. We also offer advice to family members to help them accept their loved one’s terminal diagnoses and signpost them to supporting services that can offer further help.

 

We use our effective partnerships with other end of life care services in the community including Marie Curie, Macmillan nurses, district nurses and palliative care teams to develop and review your end of life plans. We facilitate smooth transitions between support services, minimising any impact to you.

 

Where possible, our highly skilled care workers will undertake delegated tasks on behalf of district nurses and co-deliver neuro rehabilitation programmes to reduce the need for hospital appointments. Reducing this disruption prevents you and your family from becoming overwhelmed - giving you, and them space and time to focus on normal, everyday life.
