Bluebird Care Assist Health & Wellbeing Checks

The Health & Wellbeing Check is a once a week visit from one of our Care Champions. Our Champion will be able to take some vital readings along with a wellbeing questionnaire. The vital readings they will take include:

• Pulse
• Blood pressure
• Temperature
• Oxygen saturation
• Respiration rate

Bluebird Care Assist is a telehealth system that allows us to monitor your general health. Should there be any concerns it means we can get help to you quickly and preventing further illness or having to go to hospital. The frequency of checks can be tailored to health needs and are particularly of value following a stay in hospital to help reduce the chance of readmittance.
