We are looking to recruit an enthusiastic and motivated person who is keen to make a difference and help to provide the highest standards of care and support to people in their own homes. We provide care and support to people: over 65 years of age; with physical disabilities; aged 18 to 65 years; with a sensory loss or impairment; with learning disabilities or an autistic spectrum disorder; with mental health problems; with dementia; who misuse drugs or alcohol and children of all ages. Our ideal candidate will:
- Hold a minimum of Level 3 Health and Social Care or be working toward
- Have own transport
- Be passionate about providing first rate care to people in their own homes
- Have excellent verbal and written communication skills
- Be IT literate
- Have experience of carrying out assessments and drawing up person centred care plans
- Be experienced in supervising staff
- Have a flexible and ‘can do’ attitude
- Be willing to learn and undertake training
- Able to work alone and use initiative
- Be a team player
This is a full time post of 37.5 hours per week which involves some evening and weekend work as well as on call responsibilities on a rota basis. Salary is negotiable depending on qualifications and experience but within the range £18,525 to £21,450.
Duties will include:
- Carrying out customer assessments using electronic care management system
- Writing and updating care plans
- Liaising with customers and/or their representatives
- Liaising with other professionals and agencies
- Monitoring care and carrying out reviews
- Supervising and observing care staff
- Dealing with complaints
- Working with the Registered Care Manager to maintain our reputation for first class care
