Leaving hospital can feel like a huge milestone, but for many people, recovery is only just beginning. Returning home after illness, injury or surgery often comes with new challenges, especially when mobility, confidence or independence have been affected. This is where Discharge to Assess care, often known as D2A, plays an important role.

What Is Discharge to Assess?

Discharge to Assess is a short term care service designed to support people after they leave hospital. Instead of remaining in hospital while long term care needs are assessed, people can continue recovering in the comfort of their own home with professional support in place.

The goal is simple:
Help people regain as much independence as possible while ensuring they feel safe, comfortable and fully supported throughout their recovery.

D2A care is often used after:

  • Falls or injuries
  • Surgery
  • Illness or hospital treatment
  • Reduced mobility
  • Periods of rehabilitation

Every care plan is tailored to the individual and designed to support recovery at the right pace.

Why Recovering at Home Matters

For many people, home is the best place to recover. Familiar surroundings can improve wellbeing, reduce stress and help people feel more comfortable and confident during rehabilitation.

Discharge to Assess allows people to:

  • Recover in familiar surroundings
  • Rebuild confidence gradually
  • Maintain routines and independence
  • Receive personalised support
  • Avoid unnecessary extended hospital stays

Recovering at home also allows carers and enablers to support people in real life situations and routines, helping them regain confidence in everyday activities.

Supporting Independence, Not Dependence

A key part of Bluebird Care Taunton and Mid-Devon’s D2A service is supporting people to return to their baseline independence wherever possible.

This means encouraging customers to gradually rebuild confidence and complete daily tasks safely with the right level of support.

That could include:

  • Preparing meals and drinks
  • Moving safely around the home
  • Getting washed and dressed
  • Attending appointments
  • Rebuilding mobility and routines

The focus is always on progression and helping people feel more confident each day.

Real Progress That Changes Lives

One recent example of the impact of Discharge to Assess support involved Bob, who returned home after breaking his leg.

When he first left hospital, he was in significant pain, had very limited mobility and required a high level of care. Simple daily tasks felt extremely difficult.

Six weeks later, with support from Bluebird Care’s D2A enablers, Bob had made remarkable progress. When we visited him, he had already prepared his own breakfast and cup of tea independently before we arrived.

He was moving around the house more confidently, getting dressed independently and regaining control over everyday life again.

This is exactly what Discharge to Assess is designed to achieve.

Supporting Families During Recovery

Leaving hospital can also be a stressful time for families. Many people worry about whether their loved one will cope at home or recover safely without enough support.

Discharge to Assess care provides reassurance that professional help is in place while recovery progresses.

Families gain peace of mind knowing their loved one is:

  • Safe at home
  • Supported by trained professionals
  • Encouraged to regain independence
  • Receiving personalised care

The Role of D2A Enablers

Bluebird Care’s Discharge to Assess Enablers play a vital role in supporting recovery. They work closely with customers during an important stage of rehabilitation, helping them regain confidence and rebuild independence step by step.

This role is about more than care tasks. It is about encouragement, reassurance and helping people move forward positively after a difficult period.

For many enablers, seeing someone progress from needing significant support to becoming independent again is one of the most rewarding parts of the role.
