What is reablement care?

For example, they might stay close by while you prepare breakfast, support you with part of your dressing routine or help you follow movement guidance from your healthcare team.

Reablement does not mean that everyone will return to exactly how life was before. Your goals will reflect your health, abilities and what matters to you.

How can reablement care at home help?

A care professional can support you to practise or manage:

  • Moving between a bed and chair

  • Walking around your home

  • Using stairs where this has been assessed

  • Preparing meals and drinks

  • Eating and drinking

After an illness, fall or operation, you may feel less steady when standing or walking.

A care professional can offer the level of help agreed in your care plan. They can also support you to use equipment that has been assessed and recommended for you.

  • Personal care routines

  • Eating and drinking

  • Prescribed medication

  • Preparing for follow-up appointments

Our care professionals will not change your treatment or give medical advice. They’ll follow your agreed care plan and contact the right person when something needs attention.

Even after an injury has healed, you may feel nervous about moving around your home or going outside. You may worry about falling again or lose confidence in tasks you used to manage easily.

Reablement care can help you take small, supported steps.

A care professional may help you practise standing, walking with recommended equipment or moving safely between rooms.

They can also support you to rebuild familiar routines and regain confidence at your own pace.

Any new pain, weakness or concern after a fall should be discussed with a healthcare professional.
