We are delighted to be a preferred provider of End of Life Care for the Wandsworth Care Coordination Centre.
 
The Wandsworth Care Coordination Centre is a partnership between Trinity Hospice, Marie Curie and St George’s Healthcare NHS Foundation Trust. The aim of the Centre is to ensure people with a progressive, life limiting illness can access the services and support they need from one place so they can be cared for in their own home or care home.  
 
Trinity Hospice provides skilled, compassionate End of Life Care to individuals and support for families and carers in Wandsworth and across central and south west London.
 
Caring for people aged 18 years or over who have a progressive, incurable illness, many of their patients have cancer but others have long term conditions such as heart failure, lung or neurological conditions including motor neurone disease or multiple sclerosis.
 
Trinity Hospice provide many aspects of care, including an inpatient unit, outpatient services, community services and support for families and carers of patients.
 
The Patient and Family Support Team can help patients, their families and their carers to adjust emotionally and practically to the impact of the illness and continue to provide support through to bereavement.  This team provides individual counselling/therapy, art therapy, family therapy, advocacy, practical planning and children’s advice and legal work.
