At Bluebird Care Taunton & Mid-Devon, we often say that great care is about more than helping people recover. It is about helping people regain confidence, independence and a sense of normality after a challenging period in their lives.

Recently, our team received a heartfelt letter from a customer who had been supported through Pathway 1, formerly known as Discharge to Assess (D2A). Their words perfectly capture the impact that compassionate, person-centred care can have during the transition from hospital to home.

The customer wrote:

"Dear Team Bluebird, in my now awful handwriting, this is to say a heart-felt 'thank you'. I grew fond of my regular (and irregular) support team and feel great gratitude for all the kindness and encouragement I received and for the unfailing cheerfulness even when my visitor had a long and demanding day.

The transition has been easier than I had anticipated and with (redacted) now back in York (where he belongs), I am learning to get by using many of the wise precepts you and he encouraged."

What Is Pathway 1 Care?

Pathway 1, often referred to as PW1, is the NHS pathway that supports people returning home after a hospital stay when they still require short term care and rehabilitation support.

Previously known as Discharge to Assess (D2A), the service focuses on helping individuals recover safely in their own homes while promoting independence and reducing the need for long term care wherever possible.

The aim is not simply to provide support, but to help people rebuild confidence and regain the skills needed to manage daily life independently.
