Coordinating end of life care for people with dementia

Bluebird Care South Tyneside care workers are your end of life champions. We have specialist experience in coordinating and delivering end of life care for people with dementia. We’ll help to develop an up-to-date care plan that includes your end of life choices and preferences and share this with all of those involved in your care.

We work in partnership with different professionals to provide end of life care for a person with dementia, including the GP, community nurses, social workers or care home staff.

Someone in the later stages of dementia may live with their symptoms for many months, getting worse gradually. This can lead to uncertainty for you and your family. We’ll keep your family members updated as your condition changes, involving both them and you in any decisions.
