We have recently launched our Hospital Passport, which is a document that has all the important information on for our customers should they need to be transferred to hospital or supported by another care service.
We have recently launched our Hospital Passport, which is a document that has all the important information on for our customers should they need to be transferred to hospital or supported by another care service.Something that is important is the simplicity of sharing information in an appropriate way with other health professionals and our hospital passport will make what can be a fairly stressful situation just that little easier by having the appropriate information to hand.
The types of information that each customer’s Hospital Passport includes is information that must be known to keep the person safe, things important to them and information about them in general such as their likes and dislikes.
In addition to various health information, every passport includes information about hobbies, pastimes and a little history to help other professionals quickly build a picture of how best to support each individual.
